What forms should be completed?
Please submit the appropriate forms to the IRB Chair, Dr. Lamprini Pantazi, by email. Your email packet should include:
- Human Subjects Review Form A
- Depending on level of review, include either Exempt Review Form B, Expedited Review Form C, Limited Review Form D
- Consent form (additional forms if children involved)
- Flyer (if needed)
- CITI Completion Report
